The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.Key ResponsibilitiesConduct comprehensive psychosocial assessments for residents upon admission and as neededDevelop, implement, and monitor individualized treatment plansProvide individual, group, and/or family therapy sessionsAddress issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disabilitySupport residents coping with chronic illness, pain, loss of independence, and end-of-life concernsCollaborate with the Vitae clinical team, the patient's attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate careParticipate in care plan meetings and interdisciplinary team discussionsMaintain accurate and timely clinical documentation in compliance with facility, state, and federal regulationsProvide crisis intervention and behavioral management strategies when neededEducate staff and families on mental health conditions and effective communication strategiesEnsure compliance with HIPAA and ethical standards of practice
