Job Description:\n\nDirector of Human Services & Care Coordination\nDepartment: Human Services & Care CoordinationEmployment Type: Full-TimePrimary Location: St. George, South CarolinaService Area: Dorchester CountyCompensation: $62,400–$68,640 annuallyReports To: CEO/Executive Director\nJob Summary\nThe Director of Human Services & Care Coordination provides leadership and oversight for Dorchester Seniors, Inc.’s (DSI) human services, case management, care coordination, resource navigation, and rural outreach functions throughout Dorchester County.\nThe Director leads DSI’s Intervention & Stabilization Services, connecting members and community residents with resources, support, and services that promote safety, stability, independence, and quality of life. This includes overseeing services for individuals with increasingly complex needs and ensuring they are appropriately connected to DSI programs, healthcare providers, government programs, community resources, and other support systems.\nThis is a hands-on leadership position requiring a strong combination of human services knowledge, staff leadership, program management, community relationship building, compliance oversight, and organizational discipline. The Director will manage established Intervention services while helping DSI develop and strengthen Stabilization services for individuals requiring more intensive support.\nThe ideal candidate is a collaborative, compassionate, and highly organized leader who can supervise human services professionals, establish effective systems and protocols, develop strong community partnerships, monitor program performance, and expand access to services—particularly for older adults and residents living in rural and underserved areas of Dorchester County.\nThe primary office for this position is located at DSI’s St. George Center. This is not primarily an office-based position; regular field presence throughout Dorchester County is expected. Travel will include DSI’s Summerville Center, rural communities, healthcare and community partner locations, meetings, and outreach activities. A DSI vehicle will be provided for authorized business travel.\nThis position does not independently diagnose, provide clinical treatment, or make clinical determinations outside the individual’s professional scope of practice. Clinical, member-safety, compliance, and scope-of-practice concerns must be appropriately escalated to the CEO/Executive Director and/or qualified professionals.\nEssential Duties & Responsibilities\nThe responsibilities listed below represent the primary functions of this position. Responsibilities may evolve as DSI’s programs and services continue to grow.\nHuman Services & Care Coordination Leadership\n\nProvide strategic and operational leadership for DSI’s Intervention & Stabilization Services across Dorchester County.\nOversee case management, care coordination, resource navigation, rural outreach, social-service support, and related human services.\nEnsure services are responsive to member and community needs while supporting DSI’s mission and organizational priorities.\nEstablish clear standards and protocols for referrals, intake, case coordination, documentation, follow-up, escalation, and warm handoffs.\nEnsure individuals with complex needs are appropriately connected to internal DSI services and external resources.\nEstablish clear procedures for receiving referrals from DSI’s Prevention tier when member needs escalate.\nDevelop systems that allow individuals to move appropriately between DSI service tiers as their needs change.\nIdentify service gaps, emerging community needs, and opportunities to strengthen DSI’s continuum of support.\nHelp develop, implement, and strengthen DSI’s Stabilization services for individuals requiring more intensive assistance.\nEstablish workflows, procedures, documentation standards, and performance expectations necessary to support new and existing services.\nEnsure care plans, protocols, professional boundaries, and scope-of-practice requirements are documented and consistently followed.\nImmediately escalate significant compliance concerns, member-safety incidents, mandatory-reporting concerns, or scope-of-practice questions to the CEO/Executive Director.\n\nStaff Leadership & Supervision\n\nDirectly supervise the Case Manager and Rural Services Manager, as well as other Intervention & Stabilization staff assigned to the department as services develop.\nEstablish clear performance expectations, responsibilities, priorities, and measurable goals.\nConduct regular supervision, coaching, and performance-management meetings.\nProvide timely feedback and professional-development support.\nMonitor staff workloads and caseloads to promote appropriate capacity and quality service delivery.\nEnsure case records and other required documentation are accurate, complete, timely, and maintained according to established standards.\nEnsure clear protocols are followed for intake, referrals, case assignment, follow-up, escalation, and case closure.\nAddress performance, documentation, service-delivery, and compliance concerns promptly.\nPromote collaboration between DSI departments so members receive coordinated services.\nBuild a departmental culture centered on accountability, compassion, professionalism, confidentiality, collaboration, and service excellence.\n\nCase Management & Resource Navigation\n\nProvide administrative and programmatic oversight of DSI’s case-management and care-coordination functions.\nEnsure members with complex social, economic, functional, or community-support needs receive appropriate professional assistance and referrals.\nMonitor caseload activity and identify trends affecting members and service delivery.\nEnsure appropriate warm handoffs occur between DSI programs and external partners.\nSupport staff in identifying and maintaining knowledge of available community resources.\nEnsure mandatory-reporting responsibilities are understood and fulfilled.\nMaintain clear professional and organizational boundaries regarding services DSI can and cannot provide.\nEnsure situations requiring specialized expertise are appropriately referred or escalated.\n\nRural Outreach & Access\n\nLead DSI’s rural outreach strategy as an organizational priority.\nDirect the Rural Services Manager’s work to identify, engage, and connect older adults and community residents in rural and underserved areas with needed services.\nDevelop strategies to increase awareness of DSI programs and improve service access throughout western and upper Dorchester County.\nBuild relationships with faith communities, healthcare providers, community organizations, government entities, local leaders, and other organizations serving rural communities.\nIdentify opportunities for community outreach, education, referral partnerships, satellite services, and other methods of increasing DSI’s reach.\nMonitor rural outreach activity and outcomes to determine whether DSI is effectively reaching residents who may otherwise experience barriers to services.\n\nCommunity Partnerships & External Relationships\n\nDevelop and maintain a strong network of community partners supporting DSI’s human services and care-coordination work.\nBuild productive relationships with nonprofit organizations, government agencies, healthcare systems, hospitals, clinics, home-health agencies, faith communities, social-service providers, and other community resources.\nAssist with developing healthcare relationships that can serve as referral sources and collaborative partners for DSI’s current and emerging services.\nMaintain awareness of available community resources and changes that may affect DSI members.\nPromote effective two-way referral relationships with partner organizations.\nRepresent DSI at community coalitions, interagency meetings, collaborative planning groups, and other meetings as requested by the CEO/Executive Director.\nEnsure DSI’s Intervention & Stabilization Services are known and accessible to populations most likely to need them.\nRepresent DSI professionally in interactions with members, community partners, healthcare organizations, government agencies, regulators, funders, and the public.\n\nProgram Development & Continuous Improvement\n\nAssist the CEO/Executive Director with developing and implementing new human services and community-based programs.\nHelp design workflows, staffing structures, referral processes, procedures, documentation standards, and performance measures for new services.\nEvaluate existing processes and recommend improvements to strengthen service quality, efficiency, accountability, and outcomes.\nUse program data and community feedback to identify trends and opportunities for improvement.\nSupport responsible program growth by ensuring new initiatives have appropriate systems, staffing, documentation, partnerships, and oversight.\nHelp build sustainable systems that can grow with DSI rather than relying on informal or person-dependent processes.\n\nCompliance, Confidentiality & Member Safety\n\nEnsure departmental operations comply with applicable DSI policies, procedures, contracts, funding requirements, confidentiality standards, mandatory-reporting obligations, and ethical expectations.\nEnsure sensitive member information is appropriately protected.\nMonitor staff compliance with documentation and confidentiality requirements.\nEnsure departmental procedures clearly establish professional responsibilities and scope-of-practice boundaries.\nEnsure employees understand when situations must be referred to healthcare professionals, protective services, emergency services, or other qualified professionals.\nImmediately escalate significant compliance concerns, member-safety incidents, or scope-of-practice questions to the CEO/Executive Director.\nNever independently make clinical judgments on behalf of DSI unless appropriately qualified and specifically authorized to do so.\n\nDSI’s Employee Handbook establishes organizational expectations regarding Performance\n\nPrepare regular reports for the CEO/Executive Director regarding Intervention & Stabilization activity.\nMonitor and report on:\n\nCaseload and case-management activity\nReferrals received, completed, and pending\nResource-navigation activity\nRural outreach\nCommunity-partner engagement\nService utilization\nMember needs and trends\nProgram outcomes\nDepartmental performance and KPIs\n\nEnsure all required data collection and reporting are accurate, complete, and submitted on time.\nUse data to evaluate program effectiveness and inform operational decisions.\nIdentify performance or compliance concerns and develop corrective strategies.\nContribute to grant applications, funder reports, monitoring activities, and Board presentations as requested by the CEO/Executive Director.\n\nBudget & Resource Management\n\nAssist with developing and managing the departmental budget.\nMonitor departmental expenditures and use organizational resources responsibly.\nIdentify staffing, technology, equipment, partnership, and other resource needs associated with service delivery and program growth.\nAssist with grant-funded programs and funding opportunities related to human services, care coordination, rural outreach, aging services, and community-based services.\nWork collaboratively with DSI leadership to ensure programs have appropriate resources and internal controls.\n\nOrganizational Leadership\n\nServe as a member of DSI’s director-level leadership team.\nParticipate actively in leadership meetings and organizational planning.\nContribute to DSI’s strategic planning and implementation.\nCollaborate across departments to improve the member experience and organizational effectiveness.\nHelp identify emerging needs among older adults and the broader community.\nModel DSI’s organizational values and professional standards.\nSupport a culture of accountability, collaboration, continuous improvement, and service excellence.\nAttend Board of Directors meetings as directed by the CEO/Executive Director.\nPerform other duties consistent with the position as assigned.\n\nRequired Qualifications\n\nBachelor’s degree in social work, human services, public health, healthcare administration, nonprofit management, psychology, sociology, gerontology, or a related field.\nMinimum of five years of progressively responsible experience in human services, healthcare, aging services, case management, care coordination, community services, outreach, or a related field.\nAt least two years of supervisory, management, or program-leadership experience.\nDemonstrated ability to supervise employees, establish expectations, coach staff, and manage performance.\nExperience managing programs, projects, services, or community-based initiatives.\nStrong organizational and project-management skills.\nAbility to manage active programs while simultaneously developing new services, systems, or processes.\nAbility to manage or assist with management of a departmental budget.\nExcellent written, verbal, interpersonal, and relationship-building skills.\nAbility to work effectively with members, families, employees, healthcare professionals, community partners, government agencies, and other stakeholders.\nAbility to handle confidential and sensitive member information appropriately.\nAbility to understand and maintain professional and scope-of-practice boundaries.\nAbility to analyze program information, monitor performance measures, and prepare reports.\nProficiency in Microsoft Office and/or Google Workspace; experience with case-management or care-coordination software preferred.\nValid South Carolina driver’s license with no more than six points against the individual’s driving record.\nAbility to meet DSI’s requirements for authorization to operate a DSI vehicle.\nAbility and willingness to travel regularly throughout Dorchester County using a DSI-provided vehicle.\n\nDSI’s current Vehicle and Driver Policy requires annual driving-record review for employees operating DSI vehicles and prohibits an employee from operating an agency vehicle with more than six points against their driving record. \nEmployee Handbook board approved 2-8-24 (2).docx\nPreferred Qualifications\n\nMaster’s degree in social work, human services, public health, healthcare administration, nonprofit management, gerontology, or a related field.\nExperience working with older adults.\nExperience supervising case managers, care coordinators, social-service professionals, community-health workers, or outreach staff.\nExperience with CLTC, Medicaid waiver programs, home and community-based services (HCBS), or related programs.\nKnowledge of
