Occupational Therapist (OT) - Acute Care

Occupational Therapist Home Health
Schedule: 8-hour day shifts only
Requirements:
- Oregon (OR) license
- 1 year of skilled home health experience
- Basic Cardiac Life Support (BCLS) certification
Roles & Responsibilities:
- Conduct home assessments and provide evaluations for patients.
- Develop and implement individualized plans of care.
- Educate patients and their families on treatment plans, safety, and injury prevention.
- Assist patients with activities of daily living (ADL) training and transfers.
- Perform wheelchair evaluations and appropriate positioning assessments.
- Reassess patient progress and modify treatment plans as needed.
Compliance / Notes:
- Free parking available.
- Reliable transportation is expected.
- Confirm with hiring manager regarding COVID vaccination requirements.
