Role Description This is a full-time or Part-Time hybrid role for a Physical Therapist, based in Chicago, IL, with the flexibility to perform some responsibilities remotely. The Physical Therapist will assess and create individualized rehabilitation plans, provide therapeutic treatments to improve mobility and relieve pain, and collaborate with patients and families to deliver high-quality care. The role also includes documenting patient progress, coordinating effectively with other healthcare professionals, and providing guidance on continued care strategies for patients in their homes. Qualifications Strong background in Physical Therapy, Rehabilitation, and Medicine Experience in Patient Care and providing Home Care services Excellent communication and interpersonal skills to collaborate with patients, families, and healthcare teams Licensure as a Physical Therapist in the state of Illinois Experience in home health care or a similar setting is a plus Ability to work effectively both independently and within a team environment #J-18808-Ljbffr
