RN: Case Manager

Job Details: Job Title: RN: Case Manager Location: Grand Junction, CO Setting: Home Health Pay Range: 2169.20 - 2277.66 Schedule: M-F 8am-5pm (On Call Schedule: potentially one day a week, one weekend a month, but not guaranteed) Contract Length: 13 weeks Candidate Type: Traveler Requirements: Associates or Bachelor's Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Ability to analyze complex medical information and make rational decisions. Case Management certification. Responsibilities Perform comprehensive assessment of high risk members. Collaborate with primary care providers to develop and implement individualized, comprehensive care plans with specific interventions. Address, identify, and continuously reassess cost-effective, appropriate levels of care. Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals. Keep detailed records of clinical, functional, and fiscal outcomes; identify opportunities for health promotion and illness prevention. Why Choose VyTalent Solutions?
