Responsibilities Provides and ensures a safe, clean, and normalized home‑like environment at all times; informs the Program Manager of physical plant needs. Promptly informs the Program Manager/Assistant Program Manager of any serious incident or violations of resident and/or staff rights, per established policies. Develops A.D.L. (Activities of Daily Living) and ILS (Independent Living Skills) plans for residents. Maintains ongoing knowledge of each resident’s medical, psychological, cognitive, and social needs and ensures programming is appropriate to those needs and treatment goals. Maintains and submits timely and accurate records and reports regarding residents’ development status and progress, including daily goal data, monthly progress notes, and other required reports. Maintains and checks medication administration records daily as needed. Orders and manages residents’ medications in accordance with doctors’ orders. Schedules residents’ medication appointments and reviews medical records to ensure all appointments are followed. Communicates daily with nursing and management regarding residents’ appointments and follow‑ups. Participates in interdisciplinary treatment meetings and regularly scheduled resident reviews as required. Provides habilitation services to each client as prescribed in the client’s service plans (CFA, Individual Service Plan, Habilitation Plan). Assists in transporting residents to and from programs, community activities, medical appointments, home visits, and other activities as needed. Assists each client, as needed, in skill development in areas such as activities of daily living, domestic living, recreation, and socialization. Reports immediately any observed or known deficiencies or inability to provide habilitation services, as well as all types of abuse and other facility problems, including physical plant/safety issues, to management. Ensures assigned clients’ nutritional needs are met according to prescribed diet and assists with feeding as necessary. Organizes, assists, supervises, and when necessary prepares meals for residents. Assumes responsibility for agency or resident funds personally assigned for making resident or program purchases. Escorts residents to medical appointments as scheduled and completes the Staff Information Entry Form when escorting. Communicates appropriately with the Program Nurse and supervisors, both verbally and in writing (e.g., medical consult reports, incident reports, residence logbook), to ensure continuity of care and efficient operation of the residence; reviews the residence logbook daily. Assists the Housekeeper/Cook with household cleaning and facility maintenance. Performs other duties as assigned by the Program Manager, including housekeeping duties. Attendance standards for assigned shifts are at the discretion of the Program Manager or Assistant Program Manager. Successfully participates in and passes medication certification course(s) and demonstrates continued proficiency in medication administration, per established policies. Maintains awareness of and sensitivity to cultural and socioeconomic characteristics of the persons and populations served. Other essential functions of the job include, but are not limited to, the duties listed above. Qualifications H.S. Diploma or GED required; Associate’s Degree preferred with a minimum of one year experience in Human Services. Excellent oral and written communication skills; willingness to learn and use computer systems; demonstrates teamwork philosophy and professional conduct consistent with agency standards. Respect and consideration for the residents being served. Experience in medical care and interaction with medical professionals. Must pass AMAP certification training and maintain valid certification throughout employment. Valid driver’s license required (must be able to drive a large vehicle, including a wheelchair or a 15‑seat van).
