This position involves a range of responsibilities aimed at enhancing patient care and supporting recovery.

Key Responsibilities

• Deliver direct care to patients, ensuring their comfort and well-being.
• Administer prescribed medications and treatments as directed.
• Monitor and assess patient progress, reporting any changes to the supervising Registered Nurses and physicians.
• Assist in the implementation of rehabilitation plans tailored to individual patient needs.
• Work collaboratively with a multidisciplinary team to provide holistic, patient-centered care.
• Educate patients and their families about care plans and health management.
• Maintain accurate and thorough documentation in medical records.

Qualifications and Requirements

• Current Licensed Practical Nurse (LPN) license.
• Strong communication and interpersonal skills.
