Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and care plans.
• Perform comprehensive assessments, monitor vital signs, administer medications, and manage basic treatments within scope of practice.
• Develop and revise individualized care plans in collaboration with the interdisciplinary team; document changes promptly.
• Deliver wound care, IV therapy administration (where permitted), and ongoing patient education on health maintenance and safety.
• Educate patients and families on disease management, medication adherence, safety, fall prevention, and infection control.
• Communicate with supervisors, physicians, and care coordinators to coordinate services and respond to changes in patient condition; ensure accurate and timely documentation.

Requirements

• Active registered nurse license in Pennsylvania (or eligibility to practice in PA)
• Current CPR/BLS certification
• Ability to perform nursing duties in a home health setting and travel within the service area
• Strong assessment, documentation, and interpersonal communication skills
• Graduation from an accredited nursing program (ADN or BSN acceptable)
• Minimum clinical experience appropriate for home health nursing; patient-focused care mindset
