Case Coordinator

CASE COORDINATOR JOB SUMMARY: The Options for Justice Case Coordinator is responsible for providing advocacy and case coordination for individuals who have a developmental disability and are involved in the justice system, supporting the mission of the agency. They will assist individuals with eligibility determination which can open the doors to additional supports and services. The Case Coordinator will connect individuals and their families to valuable resources to promote successful navigation through the justice system and achieve desired outcomes. The Case Coordinator works collaboratively with both the criminal justice system and developmental disability service delivery systems providing education, training and support. The Case Coordinator reports directly to the Program Director and is a valuable Team member in support of agency strategic planning and development, family support, resource identification and connection. The Case Coordinator’s advocacy efforts will take place in various community settings, including court, probation offices, jails, law offices, community agencies, Options for Justice office and/or a home office. In addition to being an active Team member, this individual must also successfully work independently. DUTIES: This position is responsible for intensive and primary case coordination of all advocacy activities which includes, but is not limited to crisis aversion, referral identification and follow-up, assessment, education and training, eligibility determination, interpersonal counselling, family support, resource identification and connection. In addition, the Case Coordinator shall be responsible for individual records, reports, billing, and all pertinent agency data in accordance to agency guidelines and requirements. They will support the Program Director and Executive Director in conducting community training, education, and advocacy, always representing the agency in a positive and professional manner. The Case Coordinator will work with leadership, advisory, and community groups to continue to maintain and expand the quality of the agency’s services. Other related duties may be assigned as needed. EMPLOYMENT QUALIFICATIONS: This position requires a minimum of a Bachelor’s Degree in Criminal Justice, Social Work, Human Services or a related field in addition to five (5) years of work experience in a related field. Previous experience with case coordinator activities is helpful. Experience and/or knowledge of the justice system and disability services delivery system is preferred. Excellent written and oral communication skills are required. This staff member must be comfortable interacting with individuals and professionals in a wide variety of settings and backgrounds, be proficient in office systems and a computer, understand and follow policies and procedures, and directions. They must have excellent organizational and planning skills, manage scheduling and time effectively, be able to multi-task, and be self-driven and motivated.
