Skilled Nurse - LPN / LVN

Licensed Practical Nurse (LPN) – Home Health Join a Team That Puts Patients and Clinicians First Guardian Angel Home Care is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing Home Health team in the Akron area. As an LPN, you'll provide one-on-one skilled nursing care in patients' homes, helping them recover and maintain their independence while enjoying the flexibility and autonomy that home health offers. What We Offer Electronic charting with Homecare Homebase (HCHB) Supportive clinical leadership Independence with the backing of an experienced team Opportunities for career growth and advancement Responsibilities Provide skilled nursing care in patients' homes Assess and monitor patient conditions Administer medications and treatments as ordered Perform wound care and other skilled nursing procedures Educate patients and caregivers on medications, disease processes, and care plans Document all patient visits accurately and timely using HCHB Communicate changes in patient condition to the RN Case Manager and physician Collaborate with the interdisciplinary care team to ensure excellent patient outcomes Maintain compliance with agency, state, and federal regulations Qualifications Active Ohio Licensed Practical Nurse (LPN) license in good standing Graduate of an accredited practical nursing program Current BLS/CPR certification Valid driver's license, reliable transportation, and current auto insurance Strong communication, organizational, and documentation skills Ability to work independently in a home health environment Preferred Qualifications Previous home health or hospice experience Experience with Homecare Homebase (HCHB) or another EMR Wound care or IV therapy experience Bilingual candidates are encouraged to apply Why Guardian Angel Home Care?
