Occupational Therapist OT Home Care West Valley

Position Summary Banner Home Health occupational therapist role focuses on evaluating, planning, administering, and supervising occupational therapy treatment in accordance with physician referral, state and local standards, regulatory agencies, and departmental policies and procedures. The position demonstrates clinical competence and knowledge necessary to provide treatment appropriate to the age of patients served. Core Functions Evaluates patient’s functional abilities and limitations, determines if intervention is needed, establishes a plan of care and treatment goals, completes comprehensive evaluation within the department time frame, identifies and prioritizes key limiting factors, and collaborates with patient and family/caregiver when setting goals. Initiates discharge planning and recommends additions to or modifications of referring orders. Implements individualized treatment plan with consideration to the patient’s physical, social, spiritual, cultural, educational, and age‑specific needs, provides skilled treatment in a safe manner, assesses ongoing effectiveness of treatment, and modifies plan of care as indicated. Involves patient and family/caregiver in treatment and decision making, and provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team. Documents according to departmental and regulatory standards, documents legibly, completes documentation within the established time frames, documents innovative and individualized treatment techniques, and captures comprehensive assessment of progress or lack of progress. Treatment goals are reassessed according to department policies. Communicates pertinent patient information to ensure the best possible follow‑through of care, provides evaluation and treatment results and patient progress to physicians, nurses, patient, family/caregiver, co‑workers, and the interdisciplinary team, and contacts physicians for routine or difficult problem‑solving to ensure optimal patient outcome. Defends treatment rationale with standard practices or current literature. Exhibits adaptability in assignments related to caseload, scheduling, and staffing shifts, takes initiative to assist in other patient care areas, and accepts responsibility for all patient care needs. Demonstrates appropriate direction and delegation of personnel to achieve safe delivery of quality, cost‑effective patient care. Directs and delegates the COTA, therapy technician and student in accordance with department policies and regulatory agencies, participates in training and orientation of new personnel, and ensures competency prior to delegation of task. Participates in department or facility process improvements and meets productivity standards set by the department. Schedule Training Core: Monday‑Friday 8:00am‑5:00pm First 30‑90 days Training BSS/PRN : Commit to 1‑2 days per week Monday‑Friday 30‑60 days After Training: Option 1: Full Time 40‑32 hours per week Monday‑Friday “Work your way” 4 10hr shifts or 5 8hr shifts Saturday & Sunday + other days during the week “Work your way” 4 10hr shifts or 5 8hr shifts Commitment to working every other weekend with working weekdays on the off weeks, or working every weekend with flexibility of working one weekday. Option 2: Part Time 20‑28 hours per week Monday‑Friday: 5, 8, 10 hour days Saturday‑Sunday +1 other day 5, 8, 10 hour days Option 3: BSS/PRN PRN Position Call Outs/ PTO/ Leave Sunday‑Saturdays 8:00am‑5:00pm Minimum of 2 shifts per month Minimum Qualifications Must have successfully completed the academic requirements of an occupational therapy educational program approved by the appropriate governing board and possess a current Occupational Therapy license from the State of practice. BLS certification is required. Must demonstrate clinical competency, experience, training, and education during the interview process and must become proficient with computer software applications commonly used in an automated patient care environment. Employees must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it throughout employment.
