RN Care Manager: Coordinated Family-Centered Care

MetroPlus in New York is seeking a Registered Professional Nurse (RN) to coordinate comprehensive treatment needs for members. You will work closely with Health Home Coordinators and community agencies, providing education and support to families while participating in clinical rounds. The ideal candidate must have a Bachelor’s Degree, a valid RN license, and at least two years of clinical experience in an acute care setting.
