We are looking for dedicated and experienced Licensed Practical Nurses (LPNs) to join our growing team throughout Tampa and surrounding areas. Position Highlights LPN positions available in Tampa and surrounding areas Part-time & PRN opportunities $45–$55 per hour Flexible scheduling One-on-one patient care in the home Supportive clinical and administrative team Meaningful work with seniors and veterans Opportunity to make a direct impact on patients and families Responsibilities Provide skilled nursing care to patients in their homes according to the plan of care. Participate in ongoing assessment of patient needs and changes in condition. Complete comprehensive and ongoing patient assessments as assigned. Administer medications, injections, treatments, and other nursing interventions within the LPN scope of practice. Monitor vital signs and patient responses to treatment. Recognize changes in patient condition and communicate concerns promptly to the appropriate healthcare professional. Provide patient and caregiver education regarding medications, treatments, safety, and disease management. Document patient care accurately and timely in the EMR/EHR system. Coordinate and communicate with physicians, RNs, therapists, caregivers, and other members of the interdisciplinary team. Maintain compliance with HIPAA, agency policies, and applicable state and federal regulations. Participate in Quality Assessment and Performance Improvement (QAPI) activities. Participate in required in-services and training. Provide compassionate, respectful, and patient-centered care at every visit. Qualifications Current, unrestricted Florida LPN license Minimum 1 year of home health experience required OASIS/home-health documentation experience preferred Experience with EMR/EHR systems; Axxess experience is a plus AHCA Level II Background Screening required Current CPR certification Valid driver's license Reliable transportation Strong clinical, communication, and organizational skills Ability to work independently while maintaining excellent communication with the clinical team Compassionate and professional approach to patient care Experience Home Health: 1 year required OASIS experience preferred License & Certification Florida LPN License – Required CPR Certification – Required Valid Driver's License – Required Join UPLIFT HOMECARE At UPLIFT HOMECARE, we believe quality care starts with a strong, supported team.
