Minimum Requirements

Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Ohio PT license required to start the assignment. Minimum one year of home health (HH) physical therapy experience required. Experience documenting in HCHB (Homecare Homebase) EMR required. BLS certification from AHA or ARC may be required. Ability to perform Start of Care, recertifications, evaluations, and treatment visits in the home health setting. Preferred: Lymphedema therapy experience. Dress code: scrubs.

Assignment Details

Facility Type: Home Health. Shift Name: Days. Shift Details: Days 0800-1600. Weekly Hours: 40 (weekly hour guarantee). Coverage Areas: Travel between counties including Pickaway, Fairfield, Ross, Muskingum, and Licking as assigned. Mileage reimbursement provided between visits from first patient to last patient.
