  • Assist primary care providers in recognizing and addressing mental health and psychosocial issues as an integral member of the healthcare team.

  • Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and recommend treatment goals in collaboration with the primary care provider.

  • Deliver brief, focused therapeutic interventions for patients requiring mental health services, ensuring timely and effective care.

  • Evaluate patient needs and determine appropriate referrals to specialty mental health facilities when necessary, ensuring comprehensive care.

  • Implement follow-up procedures to monitor patient progress and adjust treatment plans as necessary to enhance outcomes.

  • Develop and assist with relapse prevention plans, facilitating patients’ maintenance of stable functionality.

  • Identify at-risk patients and develop strategic plans to prevent declines in their mental health status.
  • \n
  • Coordinate behavioral health services and link patients with external treatment providers, maintaining confidentiality and collaboration.

  • Document patient progress and diagnostic information thoroughly and accurately in the treatment chart.
  • \n
  • Communicate consistently with primary care providers about patient needs, progress, and collaborative treatment strategies.

  • Support patients in adopting healthier lifestyle choices and making behavioral changes to promote better physical and mental health.

  • Provide patients with educational resources and self-management skills necessary for active participation in their treatment.

  • Offer consultation to clinic management and team members on behavioral services, contributing to program evaluation and outcome-based improvements.

  • Assist patients in integrating any medical treatments provided by primary care providers, addressing concerns such as medication side effects.

  • Abide strictly by the Code of Ethical Conduct for Clinical Professionals and carry out additional responsibilities as assigned.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population facing a variety of mental health challenges. Our clients may include individuals experiencing anxiety, depression, relationship issues, and stress management challenges, among other psychosocial issues. Qualifications\n

  • A minimum of a Master’s Degree in Clinical Counseling.

  • Current LPC (Licensed Professional Counselor) License required.

  • Comprehensive knowledge of mental health assessment techniques, treatment planning, developmental psychopathology, and various therapy modalities including family and group therapy.

  • Familiarity with DSM-IV diagnoses and behavioral therapy methodologies.

  • Bilingual proficiency in English and Spanish is preferred but not mandatory.

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
