Licensed Practical Nurse (LPN) – Rehabilitation Job Type: Full Time | Nights Licensed Practical Nurse joining our team plays a vital role in delivering compassionate, patient‑centered care in a 40‑bed inpatient rehabilitation setting. What We're Looking For Responsibilities include: Demonstrates knowledge of medications and correct administration based on patient age and clinical condition. Reduces the potential for medication errors by following correct medication procedures. Performs patient care considering needs specific to the standard of care for patients’ age; includes geriatric and general inpatient rehabilitation patients. Under R.N. supervision, assists with the assessment of all patients and reassessments as per policy, identifies and initiates appropriate rehabilitation nursing interventions. Under R.N. supervision, performs timely and accurate QI assessments. Carries out the plan of care as indicated by the patient’s needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly. Interacts professionally with patient/family and involves patient/family in the formation of care plans. Qualifications and Requirements Graduate of an accredited licensed practical or vocational nurse program. Current state licensure as Licensed Practical/Vocational Nurse. CPR/BCLS certification; ACLS preferred. IV Certified required or obtainable within 6 months. Minimum six months medical/surgical experience in an acute care setting preferred.
