divh2Cardiology (Heart Failure) Nurse Practitioner / Physician Assistant (NP/PA)/h2pThe Cardiology Nurse Practitioner or Physician Assistant provides comprehensive cardiovascular care to patients with acute and chronic heart conditions. Working collaboratively with cardiologists and multidisciplinary healthcare teams, this role focuses on evaluation, diagnosis, treatment, and longitudinal management of cardiovascular disease using evidence-based, patient-centered practices./ppEvaluate and manage patients with cardiovascular conditions including coronary artery disease, heart failure, arrhythmias, hypertension, valvular heart disease, and lipid disorders/ppPerform comprehensive histories, physical examinations, and ongoing patient assessments/ppDevelop and implement individualized treatment plans in collaboration with supervising cardiologists/ppProvide inpatient and/or outpatient cardiology care depending on practice setting/ppInterpret diagnostic studies such as electrocardiograms, echocardiograms, stress tests, ambulatory monitoring, and laboratory results, consistent with training and privileges/ppManage cardiovascular medications and monitor therapeutic response/ppParticipate in care coordination, discharge planning, and transitions of care/ppEducate patients and families on disease processes, medications, lifestyle modification, and risk-factor reduction/ppCommunicate effectively with cardiologists, nursing staff, and consulting services/ppMaintain accurate, timely clinical documentation in the electronic medical record/ppParticipate in quality improvement, patient safety, and outcomes initiatives/ppComply with all regulatory, accreditation, and professional standards/ppGraduate of an accredited Nurse Practitioner or Physician Assistant program/ppCurrent NP or PA certification and state licensure/ppEligibility for hospital credentialing and privileging/ppDEA registration or eligibility/ppPrior experience or training in cardiology or internal medicine preferred/ppStrong clinical knowledge of cardiovascular disease management/ppExcellent assessment and diagnostic reasoning skills/ppAbility to manage acute and chronic cardiac conditions/ppStrong communication and patient education abilities/ppAbility to work collaboratively within multidisciplinary teams/ppCommitment to evidence-based practice and patient-centered care/ppOutpatient cardiology clinics, hospital inpatient units, diagnostic testing areas, or a combination of settings/ppSchedule may include clinic hours, inpatient coverage, weekends, or call depending on practice needs/ppAbility to sit, stand, and move throughout clinical environments/ppManual dexterity for examinations and clinical procedures/ppVisual and auditory acuity sufficient for patient assessment and interpretation of diagnostic data/p/div
