Physical Therapist Assistant

Sierra Home Health in Truth or Consequences is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. As a valued member of our tight-knit staff, you'll enjoy flexible scheduling, supportive leadership, and a high level of autonomy - all while serving your local community with integrity. Key Responsibilities:
- Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
- Willingness to travel to Truth or Consequences one to two days a week with mileage compensation
- Carry out treatment plans to help patients regain mobility, function, and independence
- Communicate progress and changes in patient condition to the supervising therapist
- Educate patients and caregivers on exercises and safety practices
- Document visits and progress notes accurately and on time
- Participate in care team meetings and interdisciplinary planning
- Uphold infection control and safety standards during all home visits
- Maintain professional, compassionate rapport with patients and families
- Current licensure as a Physical Therapist Assistant in New Mexico
- Previous experience in home health preferred, but not required
- Strong interpersonal and communication skills
- Confidence working independently in the field
- Adaptability to changing schedules and home environments
- Commitment to quality, safety, and patient-centered care
- Reliable transportation and valid driver's license
