This 13-week Physical Therapist assignment is based in a skilled nursing facility in Somerset, New Jersey, a community that offers a comfortable suburban feel with convenient access to the broader attractions of Somerset County and Central New Jersey. Somerset's location provides an appealing balance of quiet neighborhoods and easy commutes to larger urban centers.In this role, you will provide comprehensive Physical Therapy services to adult and geriatric residents in a SNF setting. Typical responsibilities include evaluation, development and implementation of individualized treatment plans, progression of therapeutic interventions, functional mobility and gait training, and collaboration with an interdisciplinary team to support safe and appropriate discharge planning. You will be expected to complete timely and accurate documentation within the facility's EMR and participate in care conferences as needed.The schedule is an 8-hour day shift, with a full-time weekly workload designed to support consistent patient follow-up and continuity of care. The caseload will primarily consist of short-term rehab and long-term care residents, with treatment focused on improving functional independence, reducing fall risk, and optimizing quality of life. The facility emphasizes a team-based approach to care, offering support from experienced therapists and nursing staff to help you integrate quickly and deliver high-quality rehabilitation services.
