Job Details:

• Day shift schedule with mileage reimbursement in addition to rate
• Physical Therapy licensure required in all applicable states of practice
• Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016 or foreign equivalent
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS, HS-BLS, or RQIBLS) certification from approved vendor required
• Valid Driver's License required and must be at least 21 years of age or older
• Provide physical therapy services in patients' homes
• Travel between patient locations as part of daily responsibilities
