RN - Hospice

Hospice Registered Nurse

Location: Bucks County, Montgomery County, Northeast Philadelphia

Assignment Duration: This is a 13-week contract position.

Schedule: Monday to Friday, 5 shifts per week, 8 hours per shift

Job Description: We are seeking a compassionate and dedicated Hospice Registered Nurse to join our team. The ideal candidate will have experience in hospice care, demonstrating a strong commitment to providing quality patient care. This position requires a proactive approach to managing patient needs and fostering an inviting and supportive environment for patients and their families.

Responsibilities:

• Provide patient assessments and develop care plans in coordination with the healthcare team.
• Administer medications and perform necessary medical procedures.
• Maintain accurate documentation of patient care in accordance with guidelines.
• Educate patients and families regarding their care and end-of-life options.
• Coordinate with interdisciplinary team members to ensure holistic care.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of hospice experience required.
• HCHB experience preferred.
• Current and valid RN license in hand.
