Patient Flow Clinical Coordinator Registered NurseThe Patient Flow Clinical Coordinator Registered Nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. Major duties include, but are not limited to:Accessing the Bed Management System (BMS) frequently to view available bed and to enter patients that are in transit to the medical center and have a waiting bed - and then remove the patient upon his/her arrival.Accurately entering and updating data of Veterans' condition on the Transfer Log, VISN5 Bed Board, Daily Reports, etc.Entering clinical information in CPRS and all administrative issues are to be charted in the Daily Reports - Notes Section.Great communicator, comfortable in a fast-paced environment and performance driven.Coordinating transfers with VISN5 facilities including coordinating transfer-into and out of the VA hospitals and non-VA facilities.Documenting accurate entries to the Patient Flow Report.Follows protocols to ensure timely and accurate information from referring institutions and physicians. Recommends modification in protocol based on change in patient condition.Expedites the coordination of patient's transfer with physicians, staff RN's and other departments while meeting the utilization standards, supporting a plan of care to optimize patient outcomes and decrease length of stay.Ensure appropriate level of care based on signs, symptoms and acuity of the patients using clinical skills based on quality, cost effectiveness, best practices and appropriate standards.Preparing Director's Morning Report; compile reports of data from discharges, refusals/denials.Attending and participating in Bed Huddles, Transitioning Level of Care meetings, phone conferences, collaborative meetings, phone conferences, etc.Responsible for managing communication and processes supporting all patient flow activities.Uses the Bed Management System (BMS) to view patient bed requests and movement throughout the hospital and interface with the Flow Center, clinical staff, transportation and EMS to assure patient flow occurs in a timely manner. Serves as resource to services in optimizing BMS.Performs initial admission screens using approved clinical criteria to assure patient admission and bed assignments are consistent with policy and procedure as well as clinical findings.Evaluates and negotiates appropriate patient placement, prioritizing according to current senses, staffing situations and knowledge of patient acuity.Provides oversight for timely bed assignment, triage, prioritizing admissions from different areas (direct admit from home or clinic) ensuring patients are placed in clinically appropriate level of care, using established criteria.Collaborates with the Nurse Manager using proper chain of command in managing identified delays or barriers.Leads Gemba walks, soliciting feedback from nursing and ancillary staff regarding bed availability, capacity concerns, unit/staffing concerns, and barriers to smooth patient flow.Facilitates the coordination of care and services from the ED to inpatient units.Provide support to emergency department to screen emergency department inpatient. admissions, and observations using approved criteria during covered hours.Other responsibilities, as assigned by the supervisor and required by VISN 5 directives.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Various Shifts, Weekends, and Holidays (subject to change based on needs of the Service, Facility, and/or Agency).Telework: Ad-Hoc telework available based on facility needs.Relocation/Recruitment Incentives: May not be Authorized.Permanent Change of Station (PCS): Not Authorized.
