Bilingual Case Manager - Housing & Community Support
Acacia Network
Acacia Network seeks a Case Worker to implement, monitor, and evaluate case management for individuals facing homelessness and health care barriers. You will assess needs, help secure housing, benefits, and access to services while coordinating with public agencies and community partners. You will conduct intake interviews, provide counseling for complex issues, and maintain accurate case files.
