Deliver skilled physical therapy interventions including therapeutic exercises, gait training, balance activities, edema management, and functional mobility training. Monitor patient progress, document outcomes, and adjust treatment plans to meet goals. Collaborate with interdisciplinary team (nursing, therapy, physicians, social work) to support rehabilitation and discharge planning. Ensure safety, infection control, and adherence to regulatory standards in treatment sessions and clinical documentation. Requirements Ability to work effectively in a Skilled Nursing Facility setting. Strong clinical skills in geriatric rehabilitation and functional mobility. Clear, professional communication with patients, families, and the care team. Ability to collaborate within an interdisciplinary care environment. Willingness to participate in a travel assignment and comply with facility policies.
