Primary Care Physician - Community Health

Community Health Programs Inc, a nonprofit serving the Berkshires, seeks a Primary Care Provider – Physician to deliver comprehensive, patient-centered care to diverse and underserved populations. In this team-based role, you will provide acute, chronic, and preventive primary care, manage chronic diseases, and coordinate with behavioral health, dental, and specialty services. You will use EHRs, contribute to quality and equity initiatives, and collaborate closely with an inclusive, mission-driven care team dedicated to accessible, high-quality community health.
