RN Navigator

Shift: Monday - Friday 8 am - 4:30 pm. Provides nursing care and guidance to patients from initial contact through the continuum of care. Guides patients on treatment plans, next steps in appointments, and assists with scheduling follow-up appointments. Tracks outcomes of referrals and treatments, maintaining detailed records to measure program effectiveness. Develops and implements patient education programs and tools. Provides psychosocial support, including facilitation of support groups. Educates and facilitates patient introduction to clinical trial information, resource referrals, and counseling. Advocates for patients and families in preventing and resolving clinical issues. Develops strong relationships with physicians, office staff, community resources, and healthcare providers to ensure effective communication in patient care. Serves as a liaison between clinical specialists, primary care physicians, and patients.
