Overview Clinical Liaison VIPC, Hospice Location: Glendale, CA (office-location) Position: Clinical Liaison VIPC, Hospice Position Type: Part-Time Remote/Virtual Position: No Coverage Area: Huntington Hospital / field-based compliance visits when there aren't Huntington Hospital patients Find Your Passion and Purpose as a Hospice Clinical Liaison VIPC Salary: $55 - $60 / hour Schedule: Weekend shift (20 hours) 8am-6:30pm Offer Based On Years of Experience What You Need to Know Key Responsibilities: Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in-person collaboration and communication. Serve as the primary clinical resource for hospice education, eligibility, and appropriateness. Conduct patient assessments and coordinate the admission process. Partner with Admissions, Clinical, and Care Teams for smooth transitions. Advocate for patients and families during goalofcare conversations. Participate in interdisciplinary planning, training, and outreach. Uphold ethics, quality, and continuous learning. Meet expectations for response times, referral coordination, and communication. Qualifications: One year of RN experience with knowledge of home health regulations, Medicare requirements, and careplan development. Medical sales or clinical outreach experience preferred. Strong communication and relationship‑building skills. Ability to work independently in a hospital environment. Passion for serving with dignity and empathy. Active RN license.
