Responsibilities
- Coordinate patient care from referral to discharge with physicians and the care team
- Assist with pre-authorization and facilitate consults and patient information
- Schedule consultations, diagnostics, and treatments; gather history and perform assessments
- Order labs, imaging, and other diagnostic procedures as needed
- Educate patients and families about medical care and develop educational materials
- Participate in patient rounds and case conferences; contribute to quality and safety initiatives related to wound care

Requirements
- Active Registered Nurse license in the Commonwealth of Massachusetts
- BLS certification required
- Certification in wound ostomy and continence not required
- 1–3 years related experience preferred
- Bachelor's degree in nursing preferred
- Ability to work in a hospital setting and participate in a travel assignment
