PRN Occupational TherapistRight at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life — right in the comfort of their own homes. If you're passionate about patient-centered care and love the variety that PRN work offers, we'd love to hear from you!Responsibilities:Conduct comprehensive occupational therapy evaluations and assessments for home care clientsDevelop and implement individualized treatment plans to improve clients' functional independenceProvide therapeutic interventions focused on ADLs, fine motor skills, and cognitive rehabilitationRecommend and train clients and caregivers on adaptive equipment and home modificationsCollaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinatorsMaintain accurate and timely clinical documentation in compliance with state and federal regulationsMonitor and report client progress, adjusting care plans as neededRequirements:Valid Iowa Occupational Therapist license in good standingGraduate of an accredited Occupational Therapy program (MOT or OTD preferred)NBCOT certification requiredHome health or home care experience preferredStrong clinical assessment and documentation skillsReliable transportation and valid driver's license for home visit travelExcellent communication skills and a patient, empathetic demeanorRight at Home Des Moines is a trusted provider of in-home care and assistance, dedicated to improving the lives of seniors and adults with disabilities throughout the greater Des Moines area.
