Infant Case Manager

$26 - $31.85 per hour

Case Manager, Miramar Health Center – Kennewick, WA Join a leading community health center serving over 40 clinics across Washington and Oregon. This full‑time, non‑exempt role involves building and managing a case‑management caseload, integrating services across medical, dental, behavioral health, and community support programs such as shelter, energy, and HIV counseling. Position Highlights $26.00‑$31.85 DOE with potential for higher pay for experienced candidates 100% employer‑paid health insurance (medical, dental, vision, Rx, telemedicine) Profit sharing & 403(b) retirement plan available Generous PTO, 8 paid holidays What You’ll Do Build a program‑specific caseload by seeking referrals from providers and partners. Conduct intake screenings and in‑depth client interviews to assess needs and develop service plans. Refer clients to appropriate agencies and assist with the self‑referral process. Meet regularly with clients (often in‑home) to monitor progress, provide advocacy, and resolve barriers. Maintain accurate documentation and files in accordance with program guidelines and funding regulations. Establish relationships with service providers, stay informed about community resources, and participate in interdisciplinary case reviews. Perform other duties as assigned, including clinical responsibilities aligned with TJC, HRSA, and YVFWC requirements. Qualifications Bachelor’s Degree in Social Work, Behavioral Sciences, Psychology, Child Development, or related field; OR Associate’s Degree + 2 years experience in community social services, public health, crisis intervention, outreach or linkage programs; OR High school or GED + 4 years experience in similar settings. Bilingual (English/Spanish) preferred at level 10. Valid driver’s license and proof of automobile liability insurance. Strong verbal, written, and listening communication skills. Customer‑relations skills and ability to manage stressful situations. Knowledge of medical terminology, cultural differences in care, local social and health services, and community resources. Ability to build effective relationships with providers and partners; professional interaction with diverse clients and employees. Basic proficiency with computer programs including Microsoft Outlook, Word, and Excel.
