Case Manager, Acute Care

This is a 13-week contract position.

Shift: Day 5x8-Hour (08:00 - 16:30) with 2 weekends every 6 weeks required.

We are seeking a dedicated and experienced Case Manager to join our dynamic healthcare team. The ideal candidate will have a strong background in case management along with the ability to work effectively in a fast-paced environment. This role involves coordinating care for patients, assessing medical necessity, and ensuring compliance with evidence-based guidelines.

Responsibilities:

• Coordinate and manage patient care plans, ensuring compliance with clinical standards.
• Conduct assessments and evaluations in alignment with CMS guidelines.
• Participate in interdisciplinary team meetings to develop and standardize treatment plans.
• Facilitate discharge planning, including arranging home health services and DME.
• Assist with insurance authorizations and appeal processes as needed.
• Maintain accurate documentation in the Epic charting system.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of experience in case management, preferably in an acute care setting.
• Experience with Epic or similar EMR systems is required.
• Active and valid compact RN license must be in hand at submission.
• Previous charge experience is preferred.
