The Creston Health and Rehab located in Portland, OR has an exciting opportunity available for a Full-time and PRN Physical Therapist (PT) to join the new in-house therapy team!

Qualifications:
Graduated from an accredited Physical Therapy (PT) college or university.
Valid Physical Therapy (PT) license for state practicing.
Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
Proficient in EMR documentation (we use PCC but experience in this system is not required).
CPR Certification.
