Wraparound Facilitator

SCMHCC is currently recruiting for a full-time Wraparound Facilitator. $1,000.00 sign-on bonus option. Job Summary:SCMHCC is seeking a compassionate and organized Wraparound Facilitator for our Community Based Services (CBS) program. This provider supports youth and their families by helping them access community-based mental health and support services. In this role, you will build strong relationships with families, collaborate with providers and community partners, and help ensure services are coordinated, effective, and responsive to each youth's needs. The ideal candidate is family-focused, detail-oriented, and comfortable working independently while staying connected with a multidisciplinary care team. This position offers the opportunity to make a meaningful impact by advocating for children, supporting families, and helping youth build skills for long-term stability and success. Position Responsibilities:
- Build and maintain supportive relationships with youth and their families, and advocate for their needs across service systems.
- Maintain a caseload of youth receiving community-based services who have significant emotional or behavioral needs and require care coordination to improve functioning at home, school, and in the community.
- Educate families about all available community-based services, including specific services and program expectations associated with the HCBS Serious Emotional Disturbance (SED) Waiver.
- Guide youth and families from referral through transition and discharge planning, including safety planning and connection to ongoing community and natural supports.
- Coordinate and facilitate quarterly treatment planning meetings with youth, guardians, insurance representatives, therapists, and other members of the child's support team. Meetings include setting goals, short-term objectives, strengths and needs assessment, service planning, and developing safety and crisis plans.
- Engage regularly with identified youth, SCMHCC service providers, family, and community partners involved in the youth's care (such as schools and foster agencies) to monitor progress towards treatment goals and adjust services as needed.
- Develop working knowledge of local child- and family-serving programs and resources. Connect families to community resources and referrals to create a strong support network.
- Maintain a workload that includes a minimum of 60% direct contact time with clients each week.
- Provide accurate and timely documentation in accordance with agency and program guidelines, ensuring documentation meets requirements for medical necessity, goal-directed treatment, and client response to services.
Preferred Qualifications:
- Interest in or experience working with children, adolescents, and families in human services, social services, education, behavioral health, or a related field.
- Ability to build rapport and engage children, families, and caregivers from diverse backgrounds.
- Basic understanding of family-centered or trauma-informed approaches, or willingness to learn (training provided).
- Strong verbal and written communication skills, with the ability to communicate clearly and respectfully with youth, families, providers, and community partners.
- Solid organizational skills with attention to detail and the ability to manage multiple tasks, deadlines, and client needs.
- Ability to adapt to change, manage shifting priorities, and problem-solve barriers impacting access to services.
- Comfort coordinating services across multiple systems such as schools, healthcare providers, community agencies, and insurance representatives.
- Ability to work independently in community-based settings while maintaining regular communication with supervisors and team members.
- General proficiency with computers, including Microsoft Word and Excel.
- Experience with Microsoft Teams or similar communication tools is a plus (training provided).
- Willingness to learn documentation and service coordination requirements in a regulated healthcare or social services environment.
