Candidates without these skills will not be considered for the role. Description: This position is for a registered nurse specializing in case management. The role involves providing case management services for inpatient and observation patients. It requires performing comprehensive assessments of psychosocial, medical, and discharge needs of patients and families. The case manager will coordinate the plan of care and drive the discharge plan by collaborating with a multidisciplinary team. Evaluating procedural progression using InterQual throughout the episode of care is also essential. Additional responsibilities include working collaboratively with social workers and the multidisciplinary team. The social worker will perform assessments and develop appropriate discharge plans for high-risk patients with psychosocial needs. The duties encompass consultation, discharge planning, education, counseling, advocacy, and information referrals. Candidates should possess the ability to remain calm in crisis situations. Strong communication skills to deliver information clearly and concisely are essential. A capability to build trust and act with authenticity is required. The ability to multitask and prioritize workflow and needs is necessary. Adaptability to changes in the work environment is crucial. Successful candidates will be team players. Business casual attire is required. Distinctive attributes of the unit include consistent performance metrics for the last 10 months. The team has received recognition from administrative leadership for maintaining high standards.
