Remote RN: Readmission Prevention Specialist

Readmission Prevention Coordinator (Registered Nurse) At 21st Century Home Health Services, we provide care to patients across many California counties and seek an RN to join our team. Responsibilities As a Readmission Prevention Coordinator you will analyze readmission data to identify patterns, develop evidence‑based strategies, and collaborate with clinicians to implement recommendations. You will also redesign care protocols to improve patient outcomes and reduce readmissions. Qualifications This remote position requires a valid California RN license and residency in California.
