Overview
Licensed Practical Nurse (LPN) – Long-Term Care; Utica, NY
Travel assignment starting 09/01/2026 for 13 weeks
Weekly pay: $1,432–$1,555
Guaranteed hours: 37.5 per week
Shift: 2:30 PM – 10:30 PM (evening)
Setting: Skilled Nursing Facility, long-term care unit

Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy
- Monitor residents’ conditions, vital signs, and response to therapies; document changes in the electronic medical record
- Perform routine nursing assessments; identify and escalate clinical concerns to the supervising RN
- Assist residents with activities of daily living; support mobility and safety, including fall prevention
- Collaborate with the interdisciplinary team to coordinate care and communicate with families as appropriate
- Float within the same building as needed to coverage requirements and unit needs

Requirements
- Active LPN license in New York; NURSYS verification of active licenses
- Current BLS certification
- 1+ year of experience in a long-term care (LTC) facility; LTC experience required
- EMR experience with PointClickCare
- Driver’s license with address matching demographics
- RTO (return-to-office) must be included at time of submission
- Ability to work the 2:30 PM–10:30 PM shift; 37.5 hours per week
- Local candidates within 75 miles may be considered at local rate
- Unit specifics: LTC unit with up to 40 beds; common diagnoses include MS, ortho, dementia/memory care
- Scrub color: navy blue (as dictated by unit policy)
