We are currently seeking a Clinical Pharmacist to join our exceptional team in San Antonio, Texas. The Clinical Pharmacist provides comprehensive, patient-centered ambulatory clinical pharmacy services in collaboration with physicians and other members of the healthcare team. This role will include management of physician-referred patients, development and monitoring of individualized pharmacotherapy plans, chronic disease management, patient education, medication optimization, and follow-up of an assigned patient panel. The Clinical Pharmacist will support population health initiatives through medication utilization review, prior authorization management, and analysis of pharmacy claims data to identify opportunities to improve clinical outcomes, medication utilization, and cost-effectiveness. The Clinical Pharmacist will provide services across multiple clinic locations, caring for a unique and rewarding all adult patient population of retired first responders (San Antonio Firefighters and Police) and will be expected to independently manage referrals, patient scheduling, follow-up, and ongoing medication management while maintaining effective communication with referring providers and the interdisciplinary care team. Key Responsibilities: Provides comprehensive ambulatory care pharmacy services to patients referred by internal physicians and other authorized healthcare providers. Independently manages assigned patient referrals, including patient out-reach, scheduling, assessment, medication management, documentation, and follow-up Maintains and manages an assigned patient panel, establishing appropriate follow-up intervals based on disease state, medication therapy, clinical status, and patient-specific needs. Serves as an authoritative information source on drugs and their utilization in therapy. Collects, evaluates, and interprets relevant clinical information, including laboratory values, medication history, adherence, health conditions, and patient-reported outcomes, to optimize pharmacotherapy. Develops, implements, monitors, and modifies individualized medication treatment plans in collaboration with patients and the healthcare team. Identifies medication-related problems and opportunities for optimization and communicates recommendations and treatment plans to referring providers. Identifies and makes verbal or written recommendations for specific corrective action for drug-induced problems. Provides and documents in the medical records patient education and information regarding side effects of pharmacologic therapy or treatment rationale Provides consultation on selection of appropriate dosage regimens as follow-up when non-therapeutic drug levels are obtained. Reviews medication profiles and summarizes significant findings, which are documented in the medical record and reported to the primary physician. Responsible for maintaining his/her professional competency by keeping up with information on new drugs, new drug therapies, innovations in Pharmacy practice, and integrating this information in his/her daily practices. Practices within applicable collaborative practice agreements, protocols, organizational policies, and state and federal regulations, as applicable. Diabetes & Chronic Disease Management Responsibilities: Provides medication management and patient education for patients with diabetes and other chronic disease states within the pharmacist's scope of practice. Co-leads diabetes education and disease-management classes in collaboration with the Diabetes and Lifestyle Medicine Provider. Educates patients regarding medication use, adherence, lifestyle modifications, monitoring, potential adverse effects, and therapeutic goals. Evaluates diabetes pharmacotherapy and makes evidence-based recommendations to optimize glycemic control, reduce medication-related risks, and support patient-specific treatment goals. RequirementsDoctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy. Completion of PGY1 with ambulatory experience Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or Licensed Pharmacist in another state: provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy. Preferred Qualifications: Completion of a PGY2 residency in Ambulatory Care, Pharmacotherapy, or related specialty preferred Ambulatory care pharmacy practice experience Experience managing patients under collaborative practice agreements Experience with diabetes management and/or other chronic disease states Experience with medication prior authorizations, formulary management, and utilization management Experience analyzing pharmacy claims, medication utilization, or healthcare expenditure data Experience developing or delivering patient education and group disease-management programs Health By Design
