LVN or LPN (Home Health) Shifts Available: PRN & Full Time Service Areas for Coverage:
- Northwest (Spring/Cypress, Tomball, Willowbrook, Inwood, Heights, Jersey Village)
- Northeast (Crosby, Atascocita, Kingwood, Humble)
- Southwest (59-288)
- Collaborate with the home healthcare team to develop and implement individualized care plans.
- Complete home care nursing visits.
- Prepare timely, complete, and accurate clinical notes.
- Communicate with healthcare providers and other care team members.
- Educate patients and their families about health conditions, treatment plans, and self-care techniques.
- Adhere to all agency policies, procedures, and regulatory requirements related to home health care.
- Valid and current LVN or LPN license in the state of Texas or compact state.
- Minimum of one (1) year of experience in a home health setting.
- Bilingual (Fluent in Spanish) encouraged to apply
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred but not required)
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
