Position Overview:
This full-time role provides comprehensive primary care to individuals and families, emphasizing access, continuity, and wellness. You’ll be responsible for health assessments, chronic disease management, medication management, and patient education. The practice values autonomy, team collaboration, and quality-driven care models.

You’ll work closely with other NPs, physicians, social workers, and care coordinators, with support from experienced administrative and clinical staff. The clinic emphasizes whole-person care and allows time for meaningful provider-patient relationships.

Qualifications:

• Master’s or Doctorate in Nursing (MSN/DNP)

• Certified as an FNP (FNP-C or FNP-BC)

• Licensed or license-eligible in Texas

• Bilingual in Spanish is a plus

• Strong communication and EMR proficiency
