Day Shift: 5x8-Hour (08:00 - 16:30), no weekends required.

Job Description

We are seeking a dedicated and compassionate Home Health Physical Therapist to provide exceptional care to patients within their home environments. The role entails assessing patient needs, developing personalized treatment plans, and delivering high-quality rehabilitative services.

Responsibilities

• Conduct thorough patient evaluations and assessments.
• Develop and implement individualized treatment plans based on patient needs.
• Utilize appropriate therapeutic techniques to achieve optimal patient outcomes.
• Document patient progress and treatment outcomes using EPIC Remote Client for Home Health and Rover.
• Travel to patient locations while minimizing drive times.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• Must possess a valid Oregon Physical Therapist license.
• BLS certification is required.
• Experience with OASIS documentation is preferred.
• Reliable transportation is required, capable of traveling up to 60 miles per day.
