Speech Language Pathologist (Sign-On Bonus)

Seeking a licensed Speech Language Pathologist to provide personalized care to older adults in private homes and senior living communities. Responsibilities Include Conducting evaluations and developing individualized treatment plans Delivering evidence-based therapy to support patient mobility, independence, and wellness Documenting care accurately within electronic medical records Collaborating with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists Educating patients, caregivers, and staff to promote safety and aging in place Providing telehealth and virtual care when appropriate Qualifications Valid licensure in Speech Language Pathology or eligibility Degree from an accredited program Proficiency with computers and EMR systems Current CPR certification Physical requirements include exerting moderate effort, assisting with patient transfers, and driving to care locations.
