Revisit Nurse

Hartford Healthcare is seeking a Revisit Nurse to provide high-quality follow-up care to patients across our Healthcare & Medical Services network. In this role, you will conduct focused revisit assessments, reinforce care plans, provide patient and family education, and support safe recovery after initial treatment or discharge. Working within a collaborative, team-based culture, you will coordinate with physicians and interdisciplinary staff, document in the EMR, and help prevent avoidable readmissions.
