What You'll Do

• Evaluate and screen patients with physical functioning disorders and wound care needs
• Develop and implement individualized care plans following all regulatory and clinical standards
• Administer procedures including manual techniques, therapeutic exercises, modalities, and the use of assistive devices
• Monitor patient progress, modifying treatments as needed, and document outcomes accurately
• Provide consultation and counseling to patients, families, and caregivers
• Communicate effectively with the healthcare team regarding patient status and treatment adjustments
• Support supervision of assistants, aides, or students as required

What You Need

• Valid state license as a Physical Therapist
• Experience in rehab, wound care, or long-term care settings
• Strong communication and clinical documentation skills

• Experience supervising ancillary staff or students
