Travel Physical Therapist - Skilled Nursing Facility - $3,006 per week

Overview Travel Skilled Nursing Facility (SNF) Physical Therapist in Marysville, CA. 13‑week assignment, starting April 20, 2026, with an 8‑hour night shift (40 hrs/week). Work in a SNF setting serving geriatric residents with mobility limitations, functional deficits, and varying levels of independence. Responsibilities Conduct thorough patient assessments and develop individualized treatment plans. Deliver evidence‑based PT interventions targeting strength, coordination, balance, and mobility. Collaborate closely with physicians, nursing staff, occupational and speech therapists, and family members to coordinate comprehensive patient care. Monitor and document patient progress in compliance with state, federal, Medicare/Medicaid, and facility guidelines. Participate in care planning meetings and interdisciplinary team conferences. May supervise PTAs and therapy aides as needed. Provide patient and caregiver education on safe mobility techniques, home exercises, and adaptive equipment. Requirements Graduate of an accredited Physical Therapy program. Current, active state PT license (license required if applicable). CPR certification required or preferred, depending on facility policy. Experience in an SNF or geriatric rehab setting preferred. Strong communication, documentation, and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Physical ability to lift, stand, walk, and assist patients with transfers and mobility.
