Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Perform skilled nursing assessments and document patient progress accurately
• Educate patients and family members on care procedures, medications, and health management
• Communicate changes in patient condition to supervising RN or physician promptly
• Maintain accurate and timely clinical documentation in compliance with home care regulations
• Collaborate with interdisciplinary care teams to ensure continuity and quality of care

Requirements:

• Active Florida LPN license in good standing
• Previous home care, long-term care, or clinical nursing experience preferred
• Strong knowledge of medication administration and wound care protocols
• Excellent communication, documentation, and organizational skills
• Ability to work independently with minimal supervision in a home care setting
• Valid driver's license and reliable transportation required
• CPR/BLS certification required or ability to obtain upon hire
• Compassionate, patient-focused demeanor with a commitment to quality care
