Join the Wellsville Manor Care Center team, a premier inpatient rehabilitation center, as we seek a compassionate and experienced Director of Social Work to lead our social work department. As the Director of Social Work , you will be responsible for overseeing the social work services for our residents, ensuring they receive the highest level of care and support.

Our Director of Social Work will lead a team of social workers, therapists, and caregivers to deliver individualized care plans that promote rehabilitation, independence, and overall well-being. The ideal candidate will possess exceptional leadership and communication skills, with a deep understanding of social work principles, models, and best practices.

Responsibilities

• Provide administrative leadership and supervision to the social work department, ensuring compliance with federal, state, and local regulations, as well as company policies and procedures

• Develop, implement, and evaluate social work programs and services, including counseling, case management, and community outreach

• Collaborate with the interdisciplinary team to ensure resident-centric care plans that address physical, emotional, and social needs

• Develop and maintain relationships with community-based organizations, healthcare providers, and other stakeholders to facilitate resident transitions and support

• Manage budgets, staff performance, and quality improvement initiatives to promote excellence in social work services

• Participate in performance improvement activities, including QAPI, and ensure compliance with accreditation standards

Requirements

• Master's degree in Social Work (MSW) or related field

• Current licensure as a Licensed Clinical Social Worker (LCSW) or License Master Social Worker (LMSW)

• Minimum 5 years of experience in social work, with at least 2 years in a leadership or supervisory role

• Strong communication, leadership, and organizational skills

• Ability to work in a fast-paced environment, prioritizing multiple tasks and demands

• Commitment to delivering exceptional customer service and quality care to residents, families, and colleagues
