Job Description
Title: Home Health Physical Therapist

Schedule: 8-hour day shift only

Requirements:
- Oregon physical therapy license required
- Minimum of 1 year skilled home health experience required
- Preferred experience with OASIS documentation
- Preferred skills: wound care, manual therapy
- Required certification: BCLS/BLS

Roles & Responsibilities:
- Conduct assessments and evaluations of patients in a home health setting
- Develop and implement individualized treatment plans
- Provide therapy interventions to improve patients' mobility and functional abilities
- Educate patients and caregivers on home exercise programs and safety measures
- Document patient progress and treatment outcomes in compliance with regulations

Compliance / Notes:
- Facility may cancel up to one shift per pay period
- Parking is available at no cost
- Reliable transportation is required for travel between patient visits
