Physical Therapist Lewistown, MT Specialty: PT Position Type: Travel Contract Length: 13 weeks Pay: $2042 - $2460 | Shift: Days Are you a passionate Physical Therapist looking to make a difference in the lives of patients in a home health setting? Join our healthcare partner as a Home Health Physical Therapist, where you'll play a critical role in helping patients achieve their rehabilitation goals. You'll have the opportunity to work closely with patients, their families, and other healthcare professionals to provide comprehensive care and support. Your expertise will be instrumental in assessing home safety, recommending necessary equipment, and educating patients and caregivers on effective treatment plans.
- Receive and review physician therapy orders for Home Health and Hospice patients
- Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals
- Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
- Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
- Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes
- Maintain accurate and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
- Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care
- Travel to patient homes and other care settings to provide therapy services
- Need 1 year experience in setting
- Must have current PT licensure in the state of Montana
- Must have valid driver's license
- BLS Certified
