Overview Sentara Health Plans is hiring for a Behavioral Health Care Coordinator/RN/LPC/LCSW/LMFT – Whole Person Care for Residential Facilities. This is a full-time, permanent (40 hours/week) position with a hybrid work arrangement in Central Virginia (Richmond and Petersburg). The role requires travel of up to 2 days per week to residential facilities in Central Virginia and 3 days working from home. Standard working hours are 8 AM to 5 PM EST, Monday through Friday. This position will work with members under Sentara Health Plans who are medically and behaviorally complex. The population will include children, adolescents, and adults. Position Details Position Status: Full-time, permanent (40 hours/week) Position Location: Hybrid role based in Central Virginia (Richmond, Petersburg). Travel up to 2 days/week to residential facilities; 3 days working from home. Work Schedule: 8 AM to 5 PM EST, M-F Qualifications Education Master’s degree in Counseling, Social Work, or Marriage and Family Therapy is required. For Registered Nurses, a Bachelor of Science in Nursing is required. Certification/Licensure Required license must be valid in the state of practice. One of the following licenses qualifies: Licensed Clinical Social Worker (LCSW) – Virginia (VADHP) OR LCSW – North Carolina (NCSWCLB) Licensed Professional Counselor (LPC) – Virginia (VADHP) OR LPC – North Carolina (NCBLCMHC) Licensed Marriage and Family Therapist (LMFT) – Virginia (VADHP) OR LMFT – North Carolina (NCBMFT) Registered Nurse (RN) – Virginia or North Carolina (VADHP/NCBON); Multi-State RN license where applicable Experience

• 2 years of behavioral health (Required)
• 1 year of case management (Required)
• Experience in a residential facility is preferred

Responsibilities (illustrative) • Work with medically and behaviorally complex members (children, adolescents, and adults).

• Coordinate whole-person care in collaboration with residential facilities and Sentara Health Plans.
• Travel up to 2 days per week to residential facilities; operate in a hybrid remote/work-from-home environment.
• Provide care coordination and case management support within the network.
