In this key leadership position, you will oversee the delivery of high-quality clinical care in patients' homes as well as across nursing homes and residential facilities. You will direct a high-performing home care department, leading, hiring, training, and mentoring a multidisciplinary team of nurses, physical therapists, and occupational therapists. Serving as an operational and clinical guide, you will evaluate care plans, ensure compliance, and optimize nursing interventions so our patients can thrive safely in their communities. Duties & Responsibilities

• Direct a high-performing home care department, ensuring established workflows, clinical standards, and operational policies are followed at all times.

• Guide, manage, and maintain home care staff, actively leading hiring, onboarding, training, and coaching initiatives for team members.

• Assess the quality and effectiveness of home health nursing assessments and active team participation.

• Oversee home care nurses to ensure all nursing interventions are delivered and documented accurately in alignment with patients' individualized care plans.

• Direct and monitor the quality of care provided to patients residing in nursing homes and board and care facilities.

• Oversee and arrange comprehensive multidisciplinary services, including nursing, physical therapy, and occupational therapy.

Required Qualifications

• Licensure: Active Registered Nurse (RN) License in the state of California (Required).

• Clinical Knowledge: Deep nursing expertise required to care for frail, elderly patients and successfully navigate complex clinical scenarios.

• Bachelor’s Degree preferred; additional years of relevant home care nursing experience may be substituted for education.

Desired Qualifications

• Home Health Experience: Minimum of two (2) years of nursing experience in a home health setting caring for a frail or elderly population (Preferred).

• Leadership Background: At least one (1) year of supervisory or clinical leadership experience in a Home Health environment (Preferred).

Location and Work Type

• Work Setting: On-Site / Field Operations (Clinic, Patient Homes, and Local Care Facilities).

• Schedule: Standard Daytime Hours, Monday through Friday.
